To use the From Wizard to create the simple form, first, you need to open the Database menu and then click the Form Wizard command.

Next, in the From Option select the Create a simple form and then click the Next button.

Later, you need to select the table that you want and then click the Next button.

Now, select the field that you want and click the Next button. All you need to do is to follow the wizard, step by step finish it.

Now, the Simple form appears on your screen.
